PROGRAM PARENTAL CONSENT FORM
This Parental Consent Form (“Consent Form”) is required for the participation of minor children in the Summer School Program (“Program”) organized and conducted by Bamboo Learning (“Bamboo Learning,” “we,” “us,” or “our”). By signing this Consent Form, you, as the parent or legal guardian (“Parent”), agree to the terms set forth herein concerning your child’s (“Participant”) participation in the Program.
Consent to Participate:
The Parent hereby consents to the Participant’s involvement in the Program and acknowledges that participation is voluntary. The Program includes, but is not limited to, educational classes, recreational activities, and field trips.
Acknowledgment of Risk:
The Parent acknowledges that participation in the Program may involve inherent risks, including but not limited to, physical injury, illness, emotional distress, or damage to personal property. The Parent assumes all risks associated with the Participant’s involvement in the Program.
Release and Waiver of Liability:
The Parent, on their own behalf and on behalf of the Participant, hereby releases, waives, discharges, and covenants not to sue Bamboo Learning, its officers, directors, employees, volunteers, agents, and representatives (collectively, “Releasees”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury (including death), or expense that the Participant may suffer or that may result from the Participant’s participation in the Program, whether caused by the negligence of the Releasees or otherwise.
Indemnification:
The Parent agrees to indemnify and hold harmless the Releasees from any and all liabilities, claims, damages, costs, losses, expenses, and attorney fees arising out of or in connection with the Participant’s participation in the Program, including but not limited to injuries, damages, or losses sustained by the Participant or others.
Medical Treatment Authorization:
In the event of an emergency, illness, or injury to the Participant during participation in the Program, the Parent authorizes Bamboo Learning and its representatives to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for the Participant’s immediate care and agrees that the Parent will be responsible for payment of all medical services rendered.
Insurance:
The Parent confirms that the Participant will maintain adequate health and accident insurance to cover any personal injury or illness sustained while participating in the Program. Bamboo Learning does not assume any responsibility for or obligation to provide financial assistance or other support in the event of injury or illness.
Photographic and Media Release:
The Parent grants permission to Bamboo Learning to photograph or record the Participant during the Program and to use such photographs or recordings for promotional and educational purposes without any compensation to the Participant.
Personal Property:
Bamboo Learning is not responsible for the Participant’s personal property that is lost, stolen, or damaged during the Program.
Compliance with Program Rules:
The Parent agrees that the Participant will comply with all the rules and regulations established by Bamboo Learning during the Program and that failure to do so may result in the Participant’s dismissal from the Program.
Changes to Program:
Bamboo Learning reserves the right to make changes to the Program’s schedule, activities, and staff without prior notice.
Refund Policy:
If the student misses a class, there will be no refund. If the teacher misses a class, a refund will be provided for the missed class only. If you cancel the lesson one week before the start date, 25% of the fee will be retained for damages. If you cancel during the course, no refund will be provided.
