SUMMER SCHOOL PROGRAM PARENTAL CONSENT FORM

This Parental Consent Form (“Consent Form”) is required for the participation of minor children in the Summer School Program (“Program”) organized and conducted by CentreCanada Newcomer Services Society (“CentreCanada,” “we,” “us,” or “our”). By signing this Consent Form, you, as the parent or legal guardian (“Parent”), agree to the terms set forth herein concerning your child’s (“Participant”) participation in the Program.

1. Consent to Participate:
The Parent hereby consents to the Participant’s involvement in the Program and acknowledges that participation is voluntary. The Program includes, but is not limited to, educational classes, recreational activities, and field trips.

2. Acknowledgment of Risk:
The Parent acknowledges that participation in the Program may involve inherent risks, including but not limited to, physical injury, illness, emotional distress, or damage to personal property. The Parent assumes all risks associated with the Participant’s involvement in the Program.

3. Release and Waiver of Liability:
The Parent, on their own behalf and on behalf of the Participant, hereby releases, waives, discharges, and covenants not to sue CentreCanada, its officers, directors, employees, volunteers, agents, and representatives (collectively, “Releasees”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury (including death), or expense that the Participant may suffer or that may result from the Participant’s participation in the Program, whether caused by the negligence of the Releasees or otherwise.

4. Indemnification:
The Parent agrees to indemnify and hold harmless the Releasees from any and all liabilities, claims, damages, costs, losses, expenses, and attorney fees arising out of or in connection with the Participant’s participation in the Program, including but not limited to injuries, damages, or losses sustained by the Participant or others.

5. Medical Treatment Authorization:
In the event of an emergency, illness, or injury to the Participant during participation in the Program, the Parent authorizes CentreCanada and its representatives to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for the Participant’s immediate care and agrees that the Parent will be responsible for payment of all medical services rendered.

6. Insurance:
The Parent confirms that the Participant will maintain adequate health and accident insurance to cover any personal injury or illness sustained while participating in the Program. CentreCanada does not assume any responsibility for or obligation to provide financial assistance or other support in the event of injury or illness.

7. Photographic and Media Release:
The Parent grants permission to CentreCanada to photograph or record the Participant during the Program and to use such photographs or recordings for promotional and educational purposes without any compensation to the Participant.

8. Personal Property:
CentreCanada is not responsible for the Participant’s personal property that is lost, stolen, or damaged during the Program.

9. Compliance with Program Rules:
The Parent agrees that the Participant will comply with all the rules and regulations established by CentreCanada during the Program and that failure to do so may result in the Participant’s dismissal from the Program.

10. Changes to Program:
CentreCanada reserves the right to make changes to the Program’s schedule, activities, and staff without prior notice.

11. Governing Law:
This Consent Form shall be governed by and construed in accordance with the laws of the province in which the Program is conducted.

12. Entire Agreement:
This Consent Form constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements, whether written or oral.

By agreeing to this policy, the Parent acknowledges that they have read and understood this Consent Form, agree to its terms, and are legally competent to execute it on behalf of the Participant.

YAZ OKULU PROGRAMI VELİ İZİN FORMU

Bu Veli İzin Formu ("İzin Formu"), CentreCanada Yeni Göçmen Hizmetleri Derneği ("CentreCanada," "biz," "bize," veya "bizim") tarafından düzenlenen ve yürütülen Yaz Okulu Programı’na ("Program") reşit olmayan çocukların ("Katılımcı") katılımı için gereklidir. Bu İzin Formunu imzalayarak, veli veya yasal vasi ("Veli") olarak, Katılımcı'nın Programa katılımı ile ilgili olarak burada belirtilen şartları kabul etmektesiniz.

1. Katılım İzni:
Veli, Katılımcı'nın Programa katılımını kabul eder ve katılımın gönüllü olduğunu kabul eder. Program, eğitim sınıfları, rekreasyonel aktiviteler ve saha gezileri dahil ancak bunlarla sınırlı olmamak üzere çeşitli faaliyetleri içermektedir.

2. Riskin Kabulü:
Veli, Programa katılımın kaçınılmaz riskler içerebileceğini, bunların fiziksel yaralanma, hastalık, duygusal sıkıntı veya kişisel mülkiyete zarar verme riskleri olduğunu kabul eder. Veli, Programa katılım ile ilgili olarak ortaya çıkabilecek tüm riskleri üstlenmektedir.

3. Sorumlulukten Feragat ve Waiver:
Veli, kendi adına ve Katılımcı adına, CentreCanada, onun görevlileri, yöneticileri, çalışanları, gönüllüleri, acenteleri ve temsilcileri ("Feragat Edilenler") aleyhine, Katılımcı'nın Programa katılımından kaynaklanabilecek veya bu katılımla ilgili olan her türlü yükümlülük, talep, dava, eylem ve nedenler üzerindeki tüm haklarından feragat eder ve dava açmamayı taahhüt eder.

4. Tazminat:
Veli, Feragat Edilenleri, Katılımcı'nın Programa katılımıyla bağlantılı veya bununla ilgili olarak ortaya çıkan her türlü yükümlülük, talep, zarar, maliyet, kayıp, gider ve avukat ücretleri de dahil olmak üzere her türlü sorumluluktan korumayı kabul eder.

5. Tıbbi Müdahale Yetkisi:
Acil bir durumda, Programa katılımı sırasında Katılımcı'nın hastalanması veya yaralanması halinde, Veli, CentreCanada ve temsilcilerinin, Katılımcı için gerekli görülen her türlü tıbbi tedaviyi, lisanslı bir hastane, doktor ve/ya tıbbi personel aracılığıyla sağlama yetkisine sahip olduğunu kabul eder ve tüm tıbbi hizmetlerin maliyetinden Veli sorumlu olacağını kabul eder.

6. Sigorta:
Veli, Katılımcı'nın, Program sırasında meydana gelebilecek kişisel yaralanma veya hastalığı kapsayacak yeterli sağlık ve kaza sigortasına sahip olacağını onaylar. CentreCanada, yaralanma veya hastalık durumunda mali yardım veya başka bir destek sağlama konusunda herhangi bir sorumluluk veya yükümlülük üstlenmez.

7. Fotoğraf ve Medya İzinleri:
Veli, CentreCanada'nın, Program sırasında Katılımcı'nın fotoğraflarını çekmesine veya video kaydı yapmasına izin verir ve bu fotoğrafların veya kayıtların promosyon ve eğitim amaçlı kullanılmasına karşılık herhangi bir ücret talep etmez.

8. Kişisel Mülkiyet:
CentreCanada, Program sırasında Kaybolan, çalınan veya zarar gören Katılımcı'nın kişisel mülkiyetinden sorumlu değildir.

9. Program Kurallarına Uyum:
Veli, Katılımcı'nın, Program sırasında CentreCanada tarafından belirlenen tüm kurallara ve düzenlemelere uymayı kabul eder ve uymaması durumunda Katılımcı'nın Programdan çıkarılmasına neden olabilir.

10. Programda Değişiklikler:
CentreCanada, Programın takviminde, aktivitelerinde veya personelinde önceden haber vermeden değişiklik yapma hakkını saklı tutar.

11. Uygulanacak Hukuk:
Bu İzin Formu, Programın düzenlendiği ildeki yasalarla yönetilir ve yorumlanır.

12. Tüm Anlaşma:
Bu İzin Formu, taraflar arasındaki konuyla ilgili tüm anlaşmaları içerir ve daha önce yapılmış tüm yazılı veya sözlü anlaşmaların yerini alır.

Yukarida imza atan Veli, bu İzin Formunu okuduğunu, anladığını, şartlarını kabul ettiğini ve Katılımcı adına bu formu imzalama yetkisine sahip olduğunu onaylar.
